Our Community Health Worker (CHW) Services are designed to support individuals and families by connecting them to essential health resources, addressing social determinants of health, and fostering wellness in our community. CHWs serve as trusted liaisons between residents and the services they need, ensuring that everyone has access to equitable, culturally responsive care and support.

What Are Community Health Workers ?

CHWs are compassionate advocates and resource navigators who live and work within the communities they serve. They understand the unique challenges residents face and are trained to provide education, support, and connections to services that promote health and well-being.

Our CHWs offer personalized assistance to help residents overcome barriers and access critical resources.

  • Health Education: Providing information on preventive care, managing chronic conditions, and understanding health systems.

  • Resource Navigation: Connecting residents to services such as healthcare, housing assistance, and food support.

  • Maternal and Child Health Support: Offering guidance for pregnant women and new moms, including access to prenatal care, breastfeeding support, and postpartum services.

  • Mental Health and Wellness: Promoting mental health awareness and connecting individuals to counseling and support groups.

  • Social Determinants of Health Advocacy: Addressing issues like housing, employment, transportation, and access to nutritious food.
